An 83rd minute winner from substitute Michy Batshuayi against West Brom confirms Chelsea as champions.
Chelsea beat West Brom 1-0 at the Hawthorns thanks to a late strike from Batshuayi, his first league goal since 20th August, to claim the Premier League title for the Blues.
Antonio Conte's team have been the dominant side in English football this season and their Premier League crowning was only a matter of time to many following Tottenham Hotspur's slip up against West Ham.
On a tricky night for Chelsea, it was Conte's decision to withdraw a disappionting Eden Hazard for Batshuayi which proved crucial when the 23-year-old popped up six-yards out to turn home Cesar Azpilicueta's low driven cross.
The Italian, who has had an instant impact in west London this season, is the fourth manager to lift the Premier League trophy in his first season in England with Mourinho 2004/05, Carlo Ancelotti 2009/10 and Manuel Pellegrini 2013/14.
Speaking to Sky Sports after being crown champions, captain Gary Cahill said: "We've worked so hard all season and consistently we have been the best team in the league. There is no better feeling in football."
For John Terry, who has won his fifth Premier League winners medal in a season that has seen him spend the majority of the time watching from the sidelines, it was another proud moment as club skipper. He said: "I am delighted and it has been a delight for me to sit and watch from a different perspective. The togetherness we've shown from day one, to come back and regroup after defeats to Arsenal and Liverpool and to then go on that winning streak was phenomenal."
An FA Cup final with Arsenal is still to come for Chelsea, with the opportunity for Conte to secure the double in his first season at the club and in England.
